In order for GameChef to approve your dealer application the following signed application must be completed and submitted. The application is in PDF format which requires at a miminum of Acrobat Reader 5.0. You may download a free version of this software here:

The application must be completed and signed. If you have Adobe Acrobat Standard or Acrobat Professional you may digitally sign the application and submit it by email. Otherwise you can print and fax a copy of the application, or email an unsigned copy then print and mail the completed application (the application can not be fully processed until we recieve the signed copy).
